Prepare blood sample using the draw kit as follows:

1) Please note the following blood draw requirements:

1-3 tests: 1 Full SST Tube
4-6 Tests: 2 Full SST Tubes
7 or more Tests: 3 Full SST Tubes

2) Gently invert SST 2-3 times to activate clotting enhancer.

3) Allow blood to clot for 30 minutes undisturbed.

4) Centrifuge at 1000g for 15 minutes.

5) Label the see-through plastic transport tube(s) with patient's name and transfer serum to aqua transport tube(s).

6) Place aqua transport serum tube(s) in the PVC container.

7) Place PVC container INSIDE the CLEAR LAB SEAL BAG & the Requisition Form in the OUTER POCKET.

8) Place the clear lab seal bag INSIDE the return mail-shipping box.

9) Complete the FedEx airbill and either place in a FedEx collection box or call 1-800-GO FEDEX to schedule a pick up. When FedEx asks for an account number, specify that it is a 3rd party pick-up.

10) Serum MUST be received by Immuno Labs within 10 days of draw to guarantee consistently reliable test results.

PHYSICIAN PRESCRIPTION INFORMATION (required to order testing)

Immuno Labs must receive a signed prescription in the form of a letter or a fax from the testing doctor directing Immuno Labs to send out a blood draw kit and a requisition to their patient. A FedEx return instructional sheet and FedEx label to expedite return of the specimen will also be included.

This prescription must include all the information necessary to properly fill out the requisition, which includes:

1) The ordering doctor's complete name, doctor's address & phone number

2) The patients name, address, and date of birth (DOB)

3) The physician must indicate which tests that the doctor would like preformed by our lab.

4) The prescription must also include billing information, indicating Physician, Patient, or Insurance pay.

5) VERY IMPORTANT: The doctor's signature is required on letter or fax prescription in order for Immuno Labs to complete this testing.

6)Please include diagnosis on prescription if we are to be bill insurance.
